ELIGIBLE FOR COVER BY FASIOPEN ARE COMMUNITIES OF EMPLOYEES AT COMPANIES THAT HAVE FULLY SIGNED UP TO THE FUND

The family unit eligible for cover

FasiOpen’s healthcare cover extends to members of the employee’s family unit, provided that the request is made by at least 30% of a company’s employees who are covered by the Fund.

The eligible family unit means the employee’s spouse or de facto cohabiting partner
the employee’s children
Membership requirements for children Up to the age of 18*
From 18 up to 21 years of age, provided that they currently studying on pre-university courses*
Up to 26 years of age, for the legally defined duration of their university studies*
Disabled children (67%), without age limits

* If dependent on the employee for tax purposes pursuant to current legislatio

Cover for the family unit is provided for as long as the employee is covered

Submitting a request

Requests for cover for employees, and possibly for the members of their family unit, can only be submitted by the company.
